(1) EMPLOYEE ILLNESS REPORTING AGREEMENT (FORM 1-B) IS INCOMPLETE. HAVE EMPLOYEES COMPLETE FORM 1-B (2) NO THERMOMETER WITHIN THE PEPSI-BRANDED COOLER AT CHECKOUT COUNTER. IN ADDITION, THE DISPLAY CASE COOLER AT CHECKOUT IS NOT TURNED ON. ENSURE THAT ALL COLD HOLDING UNITS ARE TURNED ON DURING THE PRE-OPENING INSPECTION AND HAVE THEM EQUIPPED WITH A THERMOMETER. (3) NO HOT WATER AVAILABLE ONSITE. THE FACILITY SHALL PROVIDE HOT WATER THAT IS AT LEAST 100F (4) THE FACILITY DOES NOT HAVE A DISPOSAL UNIT FOR OILS AND GREASE. OBTAIN THE PROPER WASTE RECEPTACLE AND SERVICES. THIS FACILITY MAY NOW STORE AND STOCK FOOD ONSITE. FURTHERMORE, OBTAIN FINAL PLUMBING INSPECTION REVIEW AND COMPLETE THE REFERRAL FORM. ONCE ALL ISSUES ARE ADDRESSED. CALL THE HEALTH DEPARTMENT FOR THE FINAL PRE-OPENING INSPECTION. |
